QA Nurse/Case Manager
- Make clinical judgments involving the observation, care, and counsel of patients.
- Administer medicines and treatments prescribed by the physician.
- Make necessary changes to the 485 Plan of Care prior to mailing (frequencies of all disciplines, teachings, orders, goals, medications, patient and physician demographic) once the coder generates it from Axxess.
- Submit the Physician Order and 485 Plan of Care for the MD’s signature. Keep track of all Orders and 485s and make certain the MD signature is within Medicare’s 30-day period.
- Track OASIS that is needed (Re-cert/Discharge/TIF/ROC) to ensure compliance with Medicare’s 30-day period.
- Review RN/LPN/CNA/Therapy visit notes for completion prior to filing in the patient’s chart. Visits must follow the frequency indicated on the 485 Plan of Care. Verify that notes are answered completely with the proper date and time of the visit and signature of the patient/PCG and visiting discipline.
- Review the 60-day Summary, Care Coordination, and Supervisory Visit of all disciplines.
- Complete lab requisitions and keep track of results for submission to the ordering physician.
- Assist/resolve field staff issues regarding patient care. Call patients to ensure the timely provision of nursing services, appropriate therapy, and DME.
- Answer/resolve phone calls regarding patient care.
- Follow-up with field staff for timely submission of notes and OASIS. Counsel field staff, if necessary, regarding frequency, OASIS accuracy, teachings, orders, etc.
- Track patients admitted to the hospital and follow up on discharge papers/orders upon their discharge.
- Approve charts for billing once all notes are completed and posted in AXXESS before handing the chart to the biller.
- Document all correspondences with physicians and other disciplines in the patient’s chart.
- Update medication profile every month and/or as needed.
Minimum Requirements: Diploma of Practical Nursing + 12mo experience in the nursing field + Passed the NCLEX-PN exam and have a current IL State license to practice practical nursing. Full time position.
